Dental Hygienists Beliefs and Intentions to Identify and Refer Patients with Disordered Eating Issues using the Social Cognitive Theory

To registered dental hygienists:
My name is Rachael Tammaro and I am pursuing a Master of Science at Forsyth School of Dental Hygiene, MCPHS University. I am requesting your participation in a survey about dental and patients with eating disorders. You will not be asked for any identifying information and all responses will be anonymous. The survey will take approximately 10-15 minutes to complete. 

You are eligible to participate in the survey if you are a registered dental hygienist with an active license. You must be over the age of 18 years, provide patient care at least 1 day per week; able to read and write in English, and able to access and the ability to complete a web-based survey.
