Predict Dental Hygienists’ Intention to Offer Anticipatory Guidance to Parents and Caregivers of Infants and Toddlers by Applying the Theory of Planned Behavior.

Hello Oregon Dental Hygienists:

My name is Melissa E. Calhoun, and I am pursuing a Master of Science at Forsyth School of Dental Hygiene, MCPHS University. I am requesting your participation in a survey to Predict Dental Hygienists’ Intention to Offer Anticipatory Guidance to Parents and Caregivers of Infants and Toddlers by Applying the Theory of Planned Behavior. You will not be asked for any identifying information and all responses will be anonymous. However, at the conclusion of the survey you will have the opportunity to click a link to learn more or have the link emailed to you. If you choose to have the link emailed to you, your email will not be linked to any survey responses.

You are eligible to participate in the survey if you meet these requirements:

You are a dental hygienist in the United States who has provided dental hygiene care to patients in at least one of the following groups in the last two years:

• expecting parents
• parents/caregivers of infants and toddlers
• children (0-5 years old)
You must also have graduated from an accredited dental hygiene program, be 18 years or older, and be able to read and write in English.
